opening TIFF image that also contains a thumbnail?

In searching the list archives, I've not yet discovered how to extract an image from a multipage TIFF that also contains a thumbnail. In this particular case, the thumbnail's IFD is the first one in the TIFF file, and it is smaller than the main image I want to pull out.

Is there a plugin or macro that can be used for this?

FWIW, we have plans to support that sort of scenario in the SCIFIO library:
https://github.com/scifio/scifio/issues/159

But it is not implemented yet.
