This issue was covered here very recently: if you increase the thickness of
the line by double clicking on the line ROI icon, you will see perpendicular
ends to it when creating new lines.

I do not think it is a good idea to add labels automatically as this will
change the behaviour of older macros and plugins and interfere with overlays
that might be already there.
Labels can be added later with a simple macro and burned into the image if
necessary.
