I'm not sure how realtime it is, but lti-civil (
http://lti-civil.org/
) has a very nice library that has been used in a plugin here:
http://www.eslide.net/civilcapture.php
. They give the source code, so you can tinker with things.

One option would be to use the Java OpenCV library:
http://code.google.com/p/javacv/
http://code.google.com/p/javacv/
With this library you can e.g. grab the frames from a webcam etc.
