This regression is fixed in the latest ImageJ daily build (1.49f4). To upgrade, use the Help>Update ImageJ command and select "daily build" from the drop down menu.

To replicate the error, I needed to:

   Open the "Confocal Series" sample stack
   Use the "c" slider to change to channel 2
   Use the "z" slider to change to slice 12
   Run Image>Stacks>Add Slice and select "Channel"
